Going Home / Discharge Planning
After your doctor has given the order for your discharge, your nurse will instruct you on your discharge needs. The discharge process can sometimes take a few hours. You may want to arrange for a family member or friend to help you in your departure from the hospital. After your discharge is completed, a staff member will escort you to the door and provide you any assistance you may need. If you have any questions about your diet, activities, or other matters, please ask.
Barb Dora, our Discharge Planner may also meet with you and/or your family as soon as your discharge needs are determined. She will help you obtain community resources such as ambulance or other transportation, admission to a rehabilitation center or skilled nursing facility, home health nursing care in your own home, access to necessary medical equipment, financial counseling, or protective services.
